Even with Promacta, will there be ITP struggles?

I was diagnosed with ITP July, 2015. For a long time, my "normal" was around 40,000. It didn't affect my energy level. I was able to keep up with an active, social lifestyle. Last February, after a bout of pneumonia, my platelets dropped to 13k, so I was put on Promacta. By being on 50 mg, my platelets are staying around 60-75k. I'm happy with that, but I am really struggling with fatigue and arthritic-like pain in my knees, elbows, and wrists. I try to do Pilates on a reformer twice a week and there are many days where all I can do is just go stretch out because my body is so tired. If there is an event (visit with grandkids, wedding, or such) I power through, but it takes days to recover. Is this normal because of ITP?? Side effect of the Promacta?? How am I suppose to know why I am feeling this way?? I am getting quite discouraged.

  • ITP since 2014. Retired nurse. My belief is empower patients to be involved as much as possible in their care. Read, read, read & ALWAYS question medics about the evidence base they use.

Known side effects of Eltrombopag I'm afraid. Over here in the UK we get patient information leaflets with our tablets explaining the likelihood of side effects. Don't you get them in the US?
A meta analysis in 2015 'Fatigue in immune thrombocytopenia' published in The British Medical Journal concluded with
The presence of fatigue has been established in a significant proportion of patients with ITP. The development of validated, reproducible, disease-specific tools for accessing HRQoL and fatigue has now paved the way for further enquiry into its pathogenesis. Studies of fatigue in chronic disease have found an association between fatigue and symptoms, such as mood and sleep disturbance, and these may be linked through inflammatory processes. The reasons for fatigue are likely to be multiple, vary between individuals and vary for an individual during the course of their illness and treatment.

It wouldn't hurt to have some Vitamin levels tested if you haven't already done that. Especially Vitamin D.
