Basic question... truly basic was created by midwest6708
I've only seen the term in writing, never pronounced out loud.
Is it "I-V-I-G"... each letter pronounced separately?
Or "I-vig", two syllables... "I" + "vig"?

Gotta know these things so as not to appear ignorant when discussing them with MDs, right?

I do know that one.
Say the letters separately. The term is an abbreviation for "It's The Pits".
