Slow response to Azathioprine was created by kievselva
Hi!
My wife had been diagnized with ITP(26.02 - 9k).
After initial attempt with prednisolone, she has started azathioprine 100 mg from 14th april, along with prednisolone 30mg which was tapered to 20mg after a week, to 15 mg after another three weeks, to 10 mg after another week, now the count is 7k.
Please advise how long it takes for Azathioprine to be effective enough to completely taper of prednisolone?
Thanks

Welcome to the "family", Kiev. I can't help with the azathioprine question as I've never had it. My question to you is, has your wife tried anything else besides that and the pred? There are other treatments available to try. I think some others will be able to help you with the azathioprine question. Good luck to your wife.

Hi Cindy!
Thanks for your response.
Yes she was prescribed cyclosporin (150g/day) when she stopped responding to pred at 50mg. But it was stopped after 4 days due to unbearable side effects (high bp every evening for 7 days accompanied by headache) may be due to the combination with pred (50mg/day).

Then pred was stopped fully and 4 day high doze dexamethazone (40mg/day) was done. On day three we got 48k, but after few days it fell again to 15k with which we flew to India.

There they did Methylprednisolone IV 1g/day for two days, before starting azathioprine (with pred). Now its a bit more than 6 weeks, Today she had 7k, from tomorrow we are back to 20mg (pred) along with azathioprine (100mg)

Thanks and good luck to you.

The pred doesn't seem to be doing anything so not much point on being on it. Azathioprine can take anything up to about three months to start working.

Hi there, I started the Azathioprine on 150mg day along with 60mg pred a day with a count of 9k. The pred didn't work at all and it took about 6wks to slowly rise by about 7k a week. Once my count was over 100k i started the pred taper.
Hope this helps, dont give up yet its a bit early. Good luck
Julia

Julia Hi!

Thanks. Of course its encouraging.

My wife is on 100 mg Azathioprine and since monday back to 20g pred.
Our hema is not very keen to increase Azathioprine. She rather increased pred to 20 from 10.
As I understand it took 6 weeks before you started responding to. It seems it takes more time for us, we are into 8th week now. We hope it she starts responding soon.
Good luck to you.
Any idea when to start tapering Azathioprine?

Thanks again
Selva
