Creative Promacta dosing or dosaging

Anybody taking Promacta in a little different way? Like skipping days? Alternating dosages etc???

I’m having problems with my counts on Promacta, even though the Med has been effective for me.

At 25 mg my count within let’s say 10-14 days will drop to 18. So, my doctor bumps it up to 37.5 mgs of Promacta. Within two weeks...my count goes from 18 to 100 or even 150.

Due to another medical issue I have, it is not advisable for me to have a count over 100. Besides, the makers of Promacta discourage counts like this.

We moved and this new doctor of mine doesn’t seem to have much experience with ITP or Promacta.

He is very uncomfortable with dosaging a little differently. Like takingvthe drig five days a week or alternating days...I’m just throwing out ideas.

I’m in a bind here. Can you tell me if you are taking your Promacta Med a little differently like skipping days and why?

It is most urgent. He seems to think it’s not done.

Thank you very much.

Nomad. Have you thought about taking 37.5 mg one day and then 25 mg the next? If you cut pills in half, one can take (25+12.5=) 37.5 mg on specific days.

  • ITP since 2014. Retired nurse. My belief is empower patients to be involved as much as possible in their care. Read, read, read & ALWAYS question medics about the evidence base they use.

In my opinion you know your body better than anyone. Alternate day dosing is quite common. If it were me, I would try it & see if it works

  • Diagnosed in 2000, at 59, after being on moderately high doses of NSAIDs for arthritis. Splenectomy and rituxan both failed (2004). Did well on prednisone till summer 2018--then terrible reactions. Promacta since 11-19.

I've only been on Promacta for 7 months, and right now I'm on 50 mg a day to boost my platelets since they dropped to 16K this week. (I'm also on a difficult pred taper, currently at 6 mg/day.) The last time I was on 50 mg a day, my platelets went too high--so my hema prescribed 25 mg every other day. Eventually, I needed 25 EVERY day--and now up to 50/day. My hema had no hesitation in prescribing the every-other-day dosage. I gather this is not uncommon.
