  • Diagnosed in 2000, at 59, after being on moderately high doses of NSAIDs for arthritis. Splenectomy and rituxan both failed (2004). Did well on prednisone till summer 2018--then terrible reactions. Promacta since 11-19.

Prednisone and prednisolone was created by karenr
I've read that prednisolone is prednisone treated to bypass some steps and that it is more easily absorbed and may cause fewer problems than prednisone. Europeans seem to talk about prednisolone; Americans apparently are almost always prescribed prednisone. Can anyone explain this to me? Would prednisolone be a better option, and is it available in the U.S.?

Prednisone is what is called a "prodrug", in that it only works after it has been converted by enzymes in the liver into prednisolone, the active form. A small number of people lack those enzymes, and for them prednisone doesn't work, while prednisolone does. I do not know what effects this might have on side effects.

  • ITP since 2014. Retired nurse. My belief is empower patients to be involved as much as possible in their care. Read, read, read & ALWAYS question medics about the evidence base they use.
