Often I read the comment that insurance companies are penny-wise and pound-foolish by not paying for Nplate or Promacta, and then having to pay expensive emergency room treatments.

Here is some ammunition for that fight with your insurance company:

Cost per Treatment Success of Thrombopoietin Receptor Agonists Vs "Watch and Rescue" Strategy for Treating Adult Non-Splenectomized Patients with Chronic Immune Thrombocytopenia: A US Payer Perspective
Conclusion:
In non-splenectomized adults with chronic ITP, the use of TPO-RAs represents an efficient way in achieving treatment success, with lower cost per treatment success than the "watch and rescue" strategy.

I always wondered that myself, but how many people really use IVIG rescue treatments enough to cost more than the TPO's? I assume they meant IVIG because no other treatment seems to fit.

I will add that I think that going through 4 or 5 treatments over 6 or 8 months only to end up with a TPO is a total waste of time and money. People are going through SO much when diagnosed and starting with TPO would make it so much easier.
