Hi , I am needing spine surgery and my surgeon said my platelets need to be over 100,000.
they were only 45k I seen a hemotoligist who gave me dexamethasone 10mg a day for 1 week they raised to 65,000 . Now he wants to start me on nplate injections once a week till they get above 100,000 then imediatley get the spine surgery . my question is does anyone know if this is a safe route to take ?
When I asked him about the side effects of nplate he told me there was no side effects at all to nplate

At one of the ITP conferences it was said to be a valid option. The only problem with Nplate is that it can take months to get stable on it.

dexamethasone 10mg a day for 1 week


That is a fairly low dose of Dex to expect anything more than what you got from it.
The usual 'Dex pulse' for most of us with ITP is 40mg/day for 4 days.
That is 10 tablets per day, each tablet being 4mg.
It is not much fun, but it works for many of us as a quick boost.
50k is generally considered the safe threshold for surgery. 100k is the new 'normal low'.

Yeah 50k is good for most surgery but not the cervical spine , at least thats what my doc told me ,
I was reading a little about nplate and was shocked to see all the side effects my dr told me there was none , also read its not safe to use to raise platelets before surgery.

I think I will be looking for a new dr ,

ok, so now Im on the prednesone and for 3 days and after using the bathroom the bowl is filled with blood and Im kinda freaking out , is this a side of the steroids ?

An oncologist at UT Southwestern Medical School told me that Nplate is effective but the major side effect of it is scar tissue within the bone marrow. I didn't know that bone marrow could have scar tissue but apparently it can. The way for that to heal is simply to get off of the nplate. Good luck. I don't envy your situation.

Yes, bone marrow reticulin and fibrosis are possible side effects, but it's rare. Many people are successfully using N-Plate with no side effects. Luckily, it is not a permanent thing.
