Does anyone have any experience with self administration of Nplate in the US after the latest FDA rule change in December 2011? My father's doctor is saying that Nplate still cannot be self injected under these rules. We know it is allowed in Europe and we're hoping the changes made would allow it here as well. Thanks, Michael

My doc (in Canada) keeps saying I will be taught how to self inject but as yet nothing. It would save a 3 hr trip once a week. I can't foresee any problems.

We got a definitive answer on this from an ITP specialist at USC medical center. You still cannot self administer Nplate, even with the less strict FDA requirements. Just part of the hoops to jump through. In the clinical trials patients self injected for 5 years, only to have to stop and go into a clinic once the drug received approval.

Puzzling that they do not let you self inject in the USA. I did it for two and a half years until it suddenly lost its efficacy.

You would have to keep the phials in a fridge whilst travelling - certainly a cool bag. I have done this myself successfully.

The dosage of my Nplate changes often depending on whether my platelets went up or down that week. It would be hard to monitor if I was injecting at home though it would be a lot more convenient.

Khary's - the dosage shouldn't be changed often based on counts. The goal is to find the lowest possible steady dose that will maintain counts around 50k. If your doctor is adjusting the dose based on your counts, he's just going to chase the counts all over the place and you won't find stability. This could also contribute to the side effects.
