Hi, this must be the place where people know best about pred side effects. If you don't mind, I'd like to ask this question: what is the exact mechanism by which prednisone irritates the stomach?

I have searched but can't zero in on the answer. Thanks.

Prednisone can irritate the stomach lining and cause reflux or ulcers, which is why it should be taken with food.

Thanks, Sandi. But do you have an idea of how that happens? I'm trying to figure out how to counteract a very bad case.

Actually, the problem is in a dog. My sister's dog. The elderly dog has an auto-immune condition called pemphigus. Treatment with pred has resulted in the dog seemingly almost at death's door, because she suddenly won't eat anything and can't even drink much without vomiting. This condition occurred after upping her dosage.

I was on this site a few years ago, when my father had ITP secondary to lymphoma. So, I thought I'd ask here, where people know from experience about pred side effects.

Ha - I had a feeling that didn't answer your question. All I know is that it can wear away the stomach lining by causing excess acid production. Aspirin and NSAID's can do the same thing.

It's odd that the dog won't eat since Prednisone increases hunger. Maybe the dog has gastritis? Has the vet checked her/him out? My dog had gastritis once and she had those same symptoms. After only one day, she needed an IV because she was dehydrated.

Thanks, I just happened to be looking into aspirin to see if there were parallels, but didn't get far. I remember reading a study a year or two ago that enteric coated aspirin didn't help, because the problem arises when the aspirin is in the blood. But apparently stomach irritation from pred is abated by taking with food as you say, so I wonder if the mechanism is the same or not. My father used to take it with yogurt.

Yes, the vet is involved but is fixated on doing a biopsy. For now, the gastritis is bad but she is able to keep down water if it's given a little at a time. So she gets water hand delivered every hour.

I was looking around here a little and see that the new ITP treatments are all TPO mimetics. Progress marches on :)
