Curious case of Neutrophil and Platlet Relationship

More

I have been seeing a pattern here-

When my neutrophils are around 4 or 5 (10'3/microL) then my platlets seem to hold up at 50k+.

when the neutrophils fall to 3 or 2 and stay there then they go below 40 requiring a shot.

Can anyone throw light on this?

Thanks,
Jay

I'm actually having the opposite problem. Platelets and neutrophils are too high. Platelets are 443 and neutrophils are 12.77. My Rheumatologist wants me to see my Hematologist but I'd rather just test again in a few weeks and see where they are. I've had wacky CBC's since the ITP diagnosis in 1998 and don't think much about it any more. I never see labs in the normal ranges. I don't know what the correlation might be with your platelets and neutrophils, but if your doctor isn't concerned, it's just one of those fluky things.

But sandi you are on prednisone so your wbc could be high because pf that.

Mine I take no steroids just nplate.

As neutrophils go up and go down platlets are going up and going down.

Yes, they always take that into account, but I've dropped my Prednisone dose and the white counts are a lot higher than they have ever been. They are usually on the low end because of Lupus. As far as platelets, I've been on higher doses of Prednisone and my counts were always around 335k. The counts went up after the dose went from 15 mg's to 10 mg's. After twelve years on Prednisone, this isn't my norm at all. She's more concerned about the neutrophils than the platelets. My lymphocyte % is low too.

Hi Jay~ I'm also on Nplate 408.5mcg weekly, so a lot higher dose than you. And my neutrophils are ALWAYS high. I think they've been high for the 3 years I've been on Nplate. It doesn't seem to have any correlation to my platelet count. I see that my neutrophils are pretty much the same whether my count is 26 or 75. My white blood count is also always high. I've wondered about this too.

Numbers for recent WBC are- 11.2 10.6 11 11.9 13.6 10.7 (normal range 4.6- 10.2)

Neutrophils -- 7.4 7.4 7.6 7.9 8.7 9.5 (normal range 2.0 - 6.9)

Posey - my reference range for neutrophils is 2.0 to 9.30. What a difference! The white count range is 4.40 to 11.30.

Maybe this could be the reason-

Migrating Platelets Are Mechano-scavengers that Collect and Bundle Bacteria

•Platelets migrate at sites of vascular injury and inflammation
•Platelets use mechanical force to scavenge substrate-bound particulate material
•Migrating platelets use this ability to collect and bundle bacteria
•Collection and bundling of bacteria facilitates neutrophil activation in sepsis

Summary

Blood platelets are critical for hemostasis and thrombosis and play diverse roles during immune responses. Despite these versatile tasks in mammalian biology, their skills on a cellular level are deemed limited, mainly consisting in rolling, adhesion, and aggregate formation. Here, we identify an unappreciated asset of platelets and show that adherent platelets use adhesion receptors to mechanically probe the adhesive substrate in their local microenvironment. When actomyosin-dependent traction forces overcome substrate resistance, platelets migrate and pile up the adhesive substrate together with any bound particulate material. They use this ability to act as cellular scavengers, scanning the vascular surface for potential invaders and collecting deposited bacteria. Microbe collection by migrating platelets boosts the activity of professional phagocytes, exacerbating inflammatory tissue injury in sepsis. This assigns platelets a central role in innate immune responses and identifies them as potential targets to dampen inflammatory tissue damage in clinical scenarios of severe systemic infection.

So in my case there is a underlying infection and the body produces more platelets and neutrophils. Hence you see my platlets and neutrophils go up.

I don't know, Jay. Your neutrophils are in the normal range, even on the low side. Neutrophils tend to go up when there is an infection. They would probably be on the higher end of the scale if you did have an infection. Low neutrophils indicate that you might be more susceptible to an infection.

So basically the relationship continues. Any time neutrophils are in between 4-5 Iam able to maintain my platlets.
It falls below 4 the platlets start dropping.

I dont know why
