Hi all......Haven't dropped in for a while. Been busy with life and all sorts of medical stuff.

I have a question. Does anyone ever get a hemorrhoid flare-up after IVIG (or any treatment) due to the increased platelet count?

Husband had dental surgery yesterday. The IVIG was Friday along with 4 days of 40 mg. of prednisone. Platelets went from 20k to 156k. (Yay! on that part.)

HOWEVER, the ONLY time he has a hemorrhoid flare-up, and I do mean major and painful, is when he has IVIG which increases his platelets. He only has IVIG prior to any surgical procedure and chooses not to treat otherwise. The last time he had IVIG for hand surgery was a couple years ago and he had a major flare then. This is not a coincidence.

His hema doctor says no but we don't agree. Was wondering about anyone else and welcome any thoughts.

Does he tend to get diarrhea or constipation after IVIG? If anything, a change in bowel habits (more frequent) would be the cause of a hemorrhoid flare and not the IVIG itself or rise in counts.

No change in bowel habits, no diarrhea, nothing different. Since he had dental surgery yesterday, everything I'm giving him comes out of a Vitamix. I would think the prednisone would have stayed in his system long enough to relieve inflammation. I don't know how fast that's eliminated. He is even on tramadol from his dental surgeon and that doesn't help. We have a hot tub and he soaked in that for a while this morning. Still, he has been in bed all day just to escape the pain and having to sit. I feel helpless and don't know what to do. I know there's nothing I can do but you know how it is.

Seriously, the exact same thing happened the last couple of times he had IVIG. No other times.

Hmmm. That is strange. The only other thing I can think of is that it's caused by the Prednisone. Steroids can cause dilated blood vessels which is exactly what hemorrhoids are. In his case, Prednisone isn't helping the inflammation, it's causing it. Maybe that, coupled with increased fluid retention from both the steroids and IVIG, are the trigger.

Thanks for that Sandi. I bet you are right on with your statement about the prednisone causing it. It's a wonder his hema didn't come up with that; but as we all know, doctors don't know everything! I guess time is the only thing that is going to take care of the problem right now.

That's the only thing that makes sense to me. It may or may not be true, but I can't think of any other explanation.

This may be TMI, but here goes...
I only suffered hemorrhoids when pregnant. My OB and every bit of conventional wisdom I found said that heat was the best remedy, but I think it made mine worse. It wasn't until I put ice on them that I found blessed relief. Suggest ice packs to your husband. Hopefully, it'll help him.

Thanks Janet. There is no such thing as TMI when it comes to medical issues. These are all things we face ourselves or with loved ones behind closed doors. These forums are to open those doors and talk about it! lol.
We had the ice discussion just this morning. I'm just gonna come right out and ask. How in the world do you get ice on hemorrhoids? There's not a lot of room between those cheeks.

LOL... That's true. It's a pretty confined space. :lol:
I don't remember specifics of how I did it ... My youngest child is 42 y.o.!
What I remember most is how great it worked.

If you can get crushed ice, put some in a small plastic zipper bag, wrap it with a damp paper towel and apply for about 20 minutes at a time. I had to lie down with my legs spread to make room ... Oh wow, now that really is TMI!
But I'm willing to sacrifice my dignity for the good of a fellow hemorrhoid sufferer.
If you have no way to crush ice, use frozen peas or other small veggies like them. They can be refrozen and reused till they're no longer needed. Put the pack inside a clean zipper bag to refreeze.
Goes without saying... Keep that package separate from your food! LOL :sick: :blush:

Thanks! I never thought of the crushed ice. Our icemaker does that. I will have him try it. One my way to WM now to get the new script for suppositories with hydrocortisone in them.
When it comes to medical issues, there is no such thing as dignity. LOL.
Anyone that's ever had a hospital gown on surely knows that!!

Well, I went to WM to pick up a box of 12 suppositories with hydrocortisone in them. The price was $449.72!!!!!!!! That's about $38 apiece!!!!! Needless to say, I didn't get them. They must have been the top of the line, dipped in 18kt gold.

If you have a compounding pharmacy in your area, call or go there and talk with the pharmacist. He/she may be able to provide such a suppository at a lower cost or recommend a custom concoction your doctor can authorize. I've known them to mix up some strange things for folks who had a need that the prepackaged stuff couldn't fulfill, such as a custom sunburn lotion recipe my local doctor used to prescribe. Good luck.

Also, were the prescribed ones much stronger than the OTC ones? Preparation H and others make some OTC ones much cheaper.

I've never seen over the counter suppositories with hydrocortisone in them. That really reduces the swelling. I'll have to check them out at the drug store and see what's available. His hema doesn't think it's related to the IVIG or prednisone. But he ONLY gets them when he has treatment. No coincidence.
