I think it might be rare for it to be enlarged. Usually the spleen is perfectly healthy, as far as I know, it just isn't acting perfectly since it is destroying perfectly healthy platelets too early. Mine is not enlarged, although my Indium scan showed my spleen was destroying 80% of my platelets within 1.5 days. I'm not expert, however.

ITP since 1989 and my spleen has never been enlarged.

I've always understood that if the spleen is enlarged they should check for something else other than ITP.

I've read the same thing as Ann. If the spleen is enlarged, they should look for another cause of low platelets, such as liver problems.

Emily I think the spleen acts perfectly normally. It is removing rogue antibodies which unfortunately are attached to platelets thus taking them out too. xx

I've also heard that in ITP the spleen should be normal on exam. My hematologist told me that last year, and my spleen continues to be normal on palpation and look normal on ultrasound.

You are right, MrsB! The spleen is doing exactly what it's supposed to do. The problem lies with the antibodies, not the spleen.
