How long does n plate take before it starts to raise platelet levels?? My platelets are in the 20's. So far we have tried IVIG, steroids, and promacta with little success. Any other ideas??

It can work right away or it can take a few weeks, depending on the dose. I'd stick with N-Plate for a while and see what happens. If that doesn't work, try Promacta again.

When you say little success do you mean no reaction to IVIG, or, it brought up counts for a week? How long did doctor give Promacta and did it bring counts up at all?

Hi Duheil - When using Nplate the doses will kind of build upon one another. This is my experience and what I've seen in others. The first week the dose often won't bring it up much- say from 7K to 15K. Then the second dose will bring it up more as it piggy-backs onto the first dose. And by the third dose the counts will usually get up to where that dose will take you. Its a tricky drug. What I learned from my hematologist- you pick a dose that will get you to 50K. Give it time to work. Then leave it alone. That will make sense as you get more experience with Nplate. Its my favorite drug for ITP- hope it works for you!

No reaction to IVIG and promacta only raised it up from 20 to 27.

I'd stick with it till the cows come home if necessary. For some folks it's almost like it has immune system regulatory powers over time.

It would have been possible to raise the dose to 75. Some people have had success with that and were able to lower the dose in time. Sometimes you have to stick with the TPO's for a while to see a response.

DUHEIL, my reaction to Nplate was immediate thankfully. My first dose 1MCL / kg took me from 74 to 120, but I also responded to IVIG and steroids very well also, hitting a high of 217 and going without treatment for 5 weeks until starting Nplate. It is a weekly injection that consumes my Monday mornings but I tolerate the dose fairly well. Yesterday I was at 143 and asked to have my dose reduced but that was denied. Best of luck! There is an interesting video on the Nplate web site you might want to watch. On a side note: I get my injection at a local cancer treatment center staffed by 5 hematologists. Yesterday I asked the Dr. how many other patients are getting Nplate here. He paused a moment, looked at the ceiling then said, just you!

That would explain his reluctance to lower the dose when your counts are above the recommended level. Hopefully, he keeps an eye on that!

I hope that was a typo '1ml/kg' and you meant 1mcg/kg

My opinion is NPlate is so expensive is it makes no sense to waste it.

I get once in 3-4 weeks 2.9 mcg\kg wait for count to be below 50.

DUHEIL, are you up to a '3' dose level of Nplate yet? That is, 3 micro-grams per kilo-gram of body weight.
