Hello everyone!!! I am pretty confused here and I was wondering if anyone would have any answers for me. So a little bit of a back story- I was diagnosed with hashimoto thyroiditis when I was about 14 or so (that an auto immune disease where my body destroys my thyroid) and within the last few years I was also diagnosed with von willebrand disease type 1 (a bleeding disorder), raynauds, and lichen sclerosus (another autoimmune disease that causes white patches on the skin). More recently my doctor noticed my wbc and platelet count was a little low. The hematologist determined by body was destroying my wbcs as well as my platelets. He thought I had lupus but the rhumatologist decided I do not. I think they're assuming my body is destroying my platelets and wbcs because of my hashimotos. So, my question is, do I have ITP? I am confused because my doctor said my body is destroying my platelets but never said anything about immune thrombocytopenia. From what i understand though, your body destroying your platelets is ITP. I'll include some lab work below incase that helps at all. Thank you sooo much to anyone who takes a look.

Not sure if this is related at all - 3 years ago when I was diagnosed with von willebrand one of the tests called platelet function interpretation said "platelet dysfunction is suggested since both studies are abnormal pre stimate" (stimate is the med used to treat von willebrand) After the stimate my platelet function was normal.

On 5/6/16 the hematologists labs said my platelet count was 119 on a range of 140-440 - The smear also said mild thrombocytopenia. Occasional large platelets present.

(also not sure if this is important but my ANA is 1:1280 with a homogeneous pattern)

In 2014 my platelet count was 144 on a range of 140-440

then on 4/6/16 (PCP's labs) it was 116 on a range of 140-440

4/6/16 WBC was 2.9 thousand on a range of 3.8-10.8
my hematocrit was also a little high as well as my lymphocytes.

neutrophils% and neutrophils,absolute were also low.


I am so sorry this information is not organized better. Please feel free to ask any questions at all. I can also upload pictures of the labs if that would be helpful. Again, thank you soooo much to anyone who takes the time to look at this. I am so confused about what is going on with me.

It's hard to say if you have ITP or not at this point. Your counts are still quite good. ITP is usually not diagnosed unless counts are consistently below 100,000. Since you have other autoimmune issues, it's also hard to say if the platelet and wbc issues are primary or secondary to another autoimmune problem.

Your ANA is pretty high, and could indicate a possible future Lupus diagnosis. I hope your Rheumatologist is going to monitor you regularly.

I wouldn't worry about the platelets right now. It is not a problem unless they drop to below 30,000.

Thank you so much! The rheumatologst didn't really say much other than I don't currently have lupus. My hematologist did say that he thinks it is likely I will develop lupus in the future since my ANA is so high and my body seems to keep destroying more and more things. I think I will see the hematologist again in a year but no plans to see the rheumatologist. I haven't even talked to my PCP about all of these tests. I am going today for what I think might be a sinus infection so maybe I will bring these tests up and see if there is anything I should be doing in the meantime. It feels so goofy to know my body is destroying so many things but there is nothing I can do- just wait until it destroys even more i suppose! Thank you sooo much for your answer! I've really been curious if thats something I would need to bring up if I got in a car accident or anything. Thanks so much again!

I have Lupus and ITP (although the ITP has been in remission). I had ITP for 8 years before being diagnosed with Lupus. I had Lupus symptoms and some elevated blood work, but didn't quite meet the criteria for about a year and a half. I was being monitored by a Rheumatologist every three months or so. Eventually, I did meet the criteria.

The best thing you can do is avoid the sun and tanning beds. UV rays can trigger Lupus antibodies.
