Is there a home test kit that can be used to monitor platelet counts similar to a diabetes tester?

Not that I have found.

I think the best one can do for home use is to buy a lab microscope, a hemocytometer, get these special buffering agent vials for measuring platelets,

www.amazon.com/Thrombo-TIC-Pure-Plus-Unopette-Replacement/dp/B004LUPA78

and learn how to do a 'manual' count as described on youtube.

No, there is nothing available for home use. Most people learn to go by symptoms anyway which can be more important than the actual count.
