CBC question about low wbc and low rbc too

Hello All,

I am pretty newly diagnosed, platelets continue to go down and are now at 49 but we are doing watchful waiting, no treatment. I have noticed since we began following early in the summer that my wbc and rbc along with hemoglobin have also been dropping. wbc has dropped then goes back up but rbc has continued to drop, not terribly low at 4.06. WBC at lowest 3.3. Dr said most likely immune problem is also causing these to be low, but not a direct immune attack like on the platelets. He diagnosed ITP. Should I be worried about Evan's syndrome?

I stopped my treatment for babesia because dr feels my thrombocytopenia could possibly be from antibiotics. My blood tests for babesia were ok but I had a clinical diagnosis and a past CDC + lyme.

Just a bit confused on wbc and rbc along with hemoglobin. I thought ITP was platelets only. Sometimes others fluctuate as well?

Hey Jen,
I also have my whites drop when my platelets are low. I guess technically it is called Evans if either reds or whites drop with platelets. My Dr never made a big deal about it, and they went back up after treatment when my platelets went up. Although he was pretty concerned when the whites went way low, and I had a raging infection. He sent me to the hospital, private room with a neutropenia sign on my door. Since I didn't feel sick at all, it was kinda fun, in a weird way. :laugh:

I wouldn't worry about it at all at this point. Your reds are in normal range...fluctuations are normal. Your whites have gone a bit below the normal range, but not too much. My CBC's are always wacky and it's never been anything to worry about.

ITP does affect platelets only. If your other counts drop consistently and become a problem, then you might need to discuss Evans Syndrome. Let your doctor worry about highs and lows. I've seen people worry needlessly many times over things that were not a problem. It's like the reference range for platelets...150k is usually the low on the reference range but someone can have counts of 110k and it's not a problem.
