I came across this article today and have no idea how I missed it in my Bloodjournal updates. It's one of the most thorough treatment articles I've ever read, listing pros and cons of three common treatments.

This article sure makes a good argument for delaying a splenectomy. It also points out the many choices we as "ITPers" have to make.

I think the article presents both sides well, but does give examples of when it is best to consider it and best not to.

I looked to see the time difference between the two articles...only 8 months. Different authors too. Oh well, safe to say they advise to wait one to three years!

I think I will re-assess my situation in December....that will be the one year point for me. However, the more I read, the more against splenectomies I become.

Here is a new article re: Rituxan and Dexamethasone. This has been published before, but the more this combination is studied, the more credible it becomes.

I had a laproscopic splenectomy after having all possible treatments attempted. It worked great for about a month. Then my platelets plummeted again. I now have an n-plate injection every week.

Well it does the job. Generally you're supposed to keep the platelets between 50,000 and 150,000, but I'm so messed up I don't actually follow all of the FDA recommendations. In general though, I'd say it's painless. You have to stay on top of it. If you miss a dose, your count will plummet. My doctor advised me to use Nplate instead of the promacta pill because apparently the pill is a bigger pain and diet restrictions. Supposedly most people dislike the pill way more than Nplate.

Goalie:

It's in your best interest to follow the Guidelines for N-Plate. Not doing so can be dangerous. Counts should actually be maintained around 50k and your doctor should be dosing you in that manner.
